Information About Your Eye Exam
Welcome to Our Office
Dr. Novalis, Dr. Worrall, and Dr. Wong are Board Certified eye physicians and surgeons specializing in problems affecting the retina (the back part of the eye). Dr. Novalis established the first retina center in Southern Arizona in 1974.
Referrals: If you belong to a HMO, you are responsible for bringing your referral with you at the time of your visit.
Copayments: Copayments are due at the time of your visit.
Items to Bring: Please bring your completed forms, picture ID (eg: driver's license), health insurance cards, a list of all your prescription medications, a driver, and dark glasses with you at the time of your visit.
Appointment Time: Your appointment allows time necessary for records processing, pupil dilation, and examination. Please plan to spend about 60-90 minutes with us during your visit, possibly longer if diagnostic studies or treatment are needed.
Pupil Dilation: It will be necessary to dilate your pupils for retinal examination. This may result in blurred vision for the rest of the day (occasionally longer). Plan to bring a DRIVER with you for your appointment, as it may not be safe for you to drive home.
Visitors: Due to the length of your visit, limited waiting room space, and the fact that delicate procedures/surgeries are being performed in the office, we ask that you bring only your driver with you to your appointment. Please do not bring children or other people to your appointment.
When you are at the office: Please do not smoke for two hours before, or during your appointment as smoking can affect your exam and studies. Food and drinks are prohibited in the office except for water. Please be sure to turn off your cell phone while you are in the office.
Cancelations: We require 24 hour notice for all cancelled appointments or you may be subject to a cancellation fee of $25. If you are ill, we prefer that you reschedule your appointment so our other patients and staff are not exposed. We will be glad to reschedule your appointment on a priority basis as soon as you are feeling well.
Se Habla Español: Our doctors and staff are multi-lingual in Spanish, French, and other languages. We also offer detailed patient education materials in different languages. If possible please bring a person with you to your appointment who is bilingual in English.
Public Transportation: Retina Centers, P.C. has 4 offices located in southern Arizona. All of our offices are on public transportation routes, are located on the first floor, and are handicap accessible.
Northwest office: Off Oracle, one block north of Orange Grove, on Suntran bus route #16. The address is 6585 N Oracle Rd, 85718.
East office: Off Grant east of Swan, in Tucson Medical Park, on Suntran bus route #9. The address is 2375 North Wyatt Dr, 85712.
Southwest office: In the Century Medical Plaza building on the first floor, located on the west side of the hospital; on Suntran bus route #3. The address is 1701 W. St. Mary's Rd, 85745.
Green Valley: Off 1-19 go east on Continental Road exit 63 to S La Canada Drive. The address is 1131 S. La Canada, 85614 in the Continental Professional Plaza.
